Community Health Needs Assessment

Hope Rising and our partners created this report in 2019 to understand the most pressing health needs in Lake County to guide planning efforts to address those needs. Special attention has been given to the needs of vulnerable populations, unmet health needs or gaps in services, and input from the communities that have a high burden of poor health factors. The assessment makes an effort to implement a transparent and collaborative approach to understanding the needs and assets in the communities with an intention to render the highest level of accountability to all partners — present and potential. Findings from this report will be used to identify, develop, and target Hope Rising Lake County’s strategies for the next three years to provide and connect residents with resources to improve health outcomes and the quality of life of residents in Lake County.
