What is a Community Health Worker?
A frontline public health worker who lives in or is trusted by the community. A CHW connects people to health and social services by breaking down barriers related to the social determinants of health (CDC.gov). They facilitate access to services and improve service delivery quality and cultural competency. Services CHWs provide include outreach, housing navigation, enhanced care management, nutrition education, advocacy, translation/interpreting, health care navigation, social support, informal counseling, tracking progress, etc. These CHWs usually share ethnicity, language, socioeconomic status and experiences with the community members they serve.
Hope Rising hosts a virtual Community Health Worker Collaborative monthly. These meetings cover the topics of CHW training, billing and reimbursement, and Partnership HealthPlan attends to answer any questions. Meetings are held on the second Friday of each month at 10:00 AM. Please email bradenck01@ah.org to join the meeting.
Community Health Worker Collaborative
Through our California Accountable Communities for Health Initiative, Hope Rising has aligned with the CalAim State Initiative. Through this initiative we have created a Community Health Worker (CHW) Collaboration. This forum brings together all CHW’s to have shared learning, support, and connection. We work closely with Partnership HealthPlan to ensure all agencies across the lake, with or interested in having CHW’s are supported in their billing and reimbursement questions.
